The role runs across two structured days each week — an in-clinic day at our Ipswich partner practice and a remote telehealth case-conferencing day — with some flexibility on the days to suit you.This is not a standard locum. You will provide clinical leadership within integrated Chronic Care Teams, working alongside allied health professionals and specialists to translate clinical findings into coordinated, evidence-based care plans.
